Introduction Acute appendicitis (AA) is considered as one of the most prevalent gastrointestinal diseases, and 5.7-57 per 100 000 individuals suffer from it annually. AA is more common among children and adolescents (1-3). Furthermore, the most common abdominal surgical emergency is appendicitis, with an estimated risk of 7% throughout life. The leading cause of the AA is claimed to be the obstruction and blockage of the proximal lumen (4). Obstruction of the appendix lumen due to several unknown factors, such as foreign body, endometriosis, appendicolith, leukemia, and intestinal parasites, are among the main mechanisms of its pathophysiology. Emotional stresses may also be considered a non-obstructive and rare etiology of appendicitis (5-7). Appendicitis and trauma may exist together, which causes an interesting debate whether trauma has led to appendicitis. We report a case of appendicitis after an abdominal trauma. Our patient developed acute appendicitis following a stab wound in the right iliac fossa. Surgical exploration confirmed the traumatic origin of appendicitis, appendectomy was performed and our patient made an excellent recovery. In non operative management of abdominal trauma, physical examinations and radiological explorations should be repeated in order to diagnose traumatic appendicitis.

Trauma has been attributed as cause for appendicitis in many cases,however isolated transection of appendix following blunt abdominal trauma is almost unheard of. We report a case of young boy who developed transection of appendix with peritonitis following cycle handle bar injury.In view of abdominal findings and CT suggestive of pneumoperitoneum,patient was taken for exploratory laparotomy.Patient had peritonitis secondary to transection of appendix leaving behind 3cm of appendiceal stump.Stump appendicectomy and lavage was done and patient had uneventful post-operative period. Hence, though rare blunt abdominal trauma may present with isolated appendiceal injury.

PURPOSE Gastrointestinal (GI) injuries in children following blunt abdominal trauma is rare; early diagnosis and treatment is important for good outcome. The purpose of this report is to describe the management problems encountered in children with GI injuries following blunt abdominal trauma. PATIENTS AND METHODS From January 1996 June 2006, 168 children were treated at our centre for abdominal trauma. Twenty three had GI injuries, 19 were due to blunt trauma while four were due to penetrating trauma. We retrospectively reviewed the clinical data of the 19 children that had GI injuries as a result of blunt abdominal trauma to document the presentation, clinical features, diagnosis and outcome. RESULTS There were 19 patients, 14 were boys, and five were girls. The median age at presentation was nine years (range 1.5 15 years). Abdominal pain is a common complaint in children, virtually; every child will have abdominal pain at some point in his life. However, most cases are mild, nonsurgical, functional, and do not need deep investigations. For example; constipation, gastroenteritis and viral syndrome are one of common causes. Nevertheless, serious causes of abdominal pain can be misleading and present with almost same complaint as per the mild causes. Acute appendicitis is the most common surgical cause of abdominal pain in children. Yet; Diagnosis is missed in 25% of cases mainly due to atypical presentation. Here we report two cases of acute appendicitis. The first case presented with a picture of acute gastroenteritis without a clear signs of acute appendicitis. In this paper, a design and development unipolar SPWM switching strategy is presented for single phase full bridge inverter. The main advantage of this strategy is that it does not required additional circuit. To obtain the unipolar SPWM switching pulses generated with carrier frequency of 2 kHz and the modulation ratio change from 0.5 to 0.7 by varying amplitude of modulating signal. In the unipolar single phase SPWM microcontroller-based 300VA inverter is designed and tested for fixed modulation index with unipolar voltage switching. The gate pulses waveforms are observed on DSO, The outputs voltage and current %THD waveforms for variable AC voltages and modulation index are observed on DSO, and also see the THD. The pulse width modulation inverter has been the main choice in power electronic for decades, because of its circuit simplicity and rugged control scheme SPWM switching technique is commonly used in industrial applications.

In caregivers of Latina breast cancer survivors, contextual factors such as informational support, Anglo-orientation, and spiritual well-being may affect physical and mental health. The purpose of this study was to test if caregiver self-efficacy moderated relationships between contextual factors and health outcomes. A model, derived from Bandura’s Social Cognitive Theory, included self-efficacy cancer knowledge (survivor) and self-efficacy symptom management (caregiver) as moderators of relationships between contextual factors and global health and depression. Secondary analysis of baseline caregiver data from an experimental study testing two psychoeducational interventions with Latina breast cancer survivors and their caregivers was conducted. Background To evaluate the safety and efficacy of intravitreal ziv-aflibercept (IVZ) in the management of vitreous hemorrhage (VH) in eyes with previously lasered proliferative diabetic retinopathy (PDR). Methods In a prospective multicenter study, previously lasered eyes who had dense VH from PDR underwent intravitreal injection of ziv-aflibercept (IVZ) (1.25 mg aflibercept). Demographic characteristics of the patients, baseline and final logMar visual acuity, number of injections, VH clearance time, and need for vitrectomy were recorded. Results Twenty-seven eyes of 21 patients were included in the study. Mean age of study patients was 61.3 ± 14.1 years with mean duration of diabetes mellitus of 22.6 ± 7.8 years.
